Immunization Information

The state of NJ requires all students to provide proof that all immunizations are up to date.

If your child is coming from another New Jersey school, you must provide a copy of the A-45. This is a school document that shows proof of Immunization.  The original will be sent to our school by the transferring school.

Students entering CRHS from out of state or country will be given a 30-day grace period to show proof of immunization. This does not apply to in-state transfers. "If after the 30 days have elapsed and no documentation of previous vaccination is provided, the child may not attend school until one dose of all age-appropriate required vaccines are received before being provisionally admitted" (NJ Dept of Health).

For Middle School that would be the completed childhood vaccinations and the updated TDap and Menactra.

If your child has religious or medical exemption, you must write a letter with the required information. Medical exemption must be updated every school year. Please see the NJ Department of Health to see what must be included in the exemption letter.
